In Diablo 4 there are technically just four tiers of items: common, uncommon, rare, and legendary. Unique items are cosmetics and armors with a unique name, like The Butcher's Cleaver, Tempest Roar, 100,000 Steps, and dozens of others. Though these items drop at a much lower rate than other endgame items, a handful of them are ridiculously rare.

The iconic two-handed sword The Grandfather returns, and so does the all-powerful Shako (called the Harlequin Crest), alongside four other ultra-rare Uniques. Despite their extremely low drop rate, there is a method to increase one's chances of getting one of these six rarest Diablo 4 items – but keep your expectations low.

How To Get The Rarest Diablo 4 Uniques

Players taking on World Tier 3 and 4 are likely knee-deep in legendaries and Uniques, but their drop rates aren't quite as random as one might think. It's possible to target certain enemy types for different drops – for example, Goatmen have a slightly higher chance to drop Totems, which can be useful for Druids hunting down Druid-specific Aspects.

Unfortunately, this doesn't apply to the six rarest Diablo 4 items.

There is only one way to target farm these rare unique Diablo 4 items; As pointed out by Lead Class Designer Adam Jackson, to get these items, players must be at least Level 85 in Nightmare difficulty (World Tier 4). These six rare Uniques won't drop for lower-level players. Players must simply complete content that rewards Uniques, like Helltide Mystery Chests, Nightmare Dungeons, and other endgame activities.

That's all there is to it, but the actual drop rate of these items is so astronomically low that many players wonder if they have better chances of winning the lottery. The first Harlequin Crest (Shako) was found on June 11, 2023, around a week and a half after the game's early access launch – and the first drop of The Grandfather came June 28, 2023, nearly a month after release.
