While Sanctuary has always existed since the first game, Diablo 4 provides the first real opportunity for players to come face to face with Lilith, one of its co-creators. It was her union with Inarius that gave rise to the land, and while they have been at odds ever since, her relationship with the people, Inarius, and the Eternal Conflict is still interesting fare.

Her plans for ending the reign of angels and demons alike may have merit, but it requires the players not to stand in the way. That is not what is going to happen, and as such, a titanic battle awaits.

As the final big boss of Diablo 4, it makes perfect sense for Lilith to be the most challenging. All preparation must be made, so be sure to visit the Blacksmith to upgrade all the gear and equip both offensive and defensive skills that will come in handy for a prolonged fight.

At the beginning, Lilith will attack the player in her Creator of Sanctuary form, which is what is usually shown when she is on-screen in Diablo 4. This means she is fast and deadly, with the following attacks to look out for:

  • An air smash move will hit a large area for significant damage, so avoid the radius when Lilith goes off-screen.
  • Lilith will also attack with long lines of flames, alongside melee attacks, to keep players on their toes. Keep moving to avoid any damage and counterattack.

A good way to gain an advantage is to use crowd control attacks on her, building up the stagger bar and opening up Lilith for sustained damage without any reprisal. This first stage is relatively easy to get past, and once her health is gone, she will be back for more.

Lilith comes back with a vengeance in her Daughter of Hatred guise and becomes much more aggressive than before. She retains all of her previous attacks, albeit enhanced to dangerous levels, and her AoE effects are larger as well. Blood orbs will also appear on the ground, so avoid them as well.

There is also the possibility of being one-shotted by Lilith, so be agile in order to avoid any unnecessary damage when facing off against her. Reducing her health will also cause Lilith to destroy parts of the arena using a powerful AoE attack, slowly reducing the space with which players can work with. Ration the use of healing potions, consider using Elixirs, and attack whenever possible.

Eventually, not even Lilith can stand in the way of Sanctuary's heroes, and she will fall. Collect all of that valuable loot, and get ready for more endgame systems in Diablo 4, such as the Whispers of the Dead, Helltide events, and Nightmare Dungeons. The journey has only just begun.
