Among all the loot Diablo 4 players have a chance to find, the rarest are the unique quality items offering special affixes that can't be found on any other piece of gear. As such, players may need to spend some time farming for the right piece of unique gear to complete their class' build. However, some of these uniques are much better than others, and the Barbarian class in Diablo 4 has a variety of uniques that are useful and others that don't hold up compared to other options available.

Unique quality items have a chance to drop beginning at Diablo 4's World Tier 3, which means these items are intended for late-game builds as players can't access World Tier 3 until they complete the main quest and take on the Cathedral of Light Capstone Dungeon. By this point in the game, players will want to be optimizing their build, so it is important to understand which uniques work best for which types of build, and which are not worth the time to grind for. While most uniques are solid, some are more versatile than others that are geared towards a more niche build.

S-Tier

An armored Barbarian in Diablo 4

Rage of Harrogath – The most versatile unique for Diablo 4 Barbarians is the chest piece Rage of Harrogath. While it doesn't offer affixes geared towards a specific skill, it does come with a fantastic unique affix that has the chance to reduce the cooldowns of all non-Ultimate skills by one-and-a-half seconds when inflicting Bleeding on an Elite enemy. Aside from the fact that this effect can be great for keeping the damage output rolling, it also adds Thorns to the player and increases Physical damage among other useful effects to help the player survive longer against a horde of enemies.

A-Tier

Gohr's Devastating Grips – The Barbarian's Whirlwind build is one of the class' best options in Diablo 4, and these unique gloves fit best on this type of build. Gohr's Devastating Grips help enhance the power of Whirlwind by not only granting it additional ranks but offering a unique affix that causes Whirlwind to explode when it ends, dealing a percentage of the base damage as Fire damage to nearby enemies. It also has affixes to help boost the Barbarian's Fury generation and attack speed making it a go-to option for Whirlwind users.

Overkill – The unique two-handed mace Overkill is a great option for players taking advantage of the Barbarian's Death Blow skill as this weapon provides additional ranks to the skill on top of a unique affix that creates a shock wave when Death Blow is used which contributes the ability's cooldown reset when it kills an enemy. It also comes with affixes that deal increased damage and offer a higher critical chance to Injured enemies.

Ramaladni's Magnum Opus – Ramaladni's Magnum Opus is a solid one-handed sword that can find a place on most Diablo 4 Barbarian builds. Its unique affix slightly increases damage per point of Fury the player has, which can add up to a significant power boost. However, it also drains the player of two Fury points every second, so they will need to be constantly generating the resource to make this weapon worthwhile.

B-Tier

100,000 Steps – The 100,000 Steps unique boots are a solid piece of gear that can be a bit of a hassle to use optimally. Their unique affix allows the Barbarian to automatically cast the Ground Stomp skill every 30 seconds for additional Fury, but requires players to first receive all three damage bonuses from the Walking Arsenal skill. This can be a great item for players who are constantly swapping weapons using the Barbarian's Arsenal mechanic as it also increases damage done when swapping weapons, but the need to invest in multiple skills and stick to a specific play style makes it ultimately a fairly niche unique.

Fields of Crimson – While still a decent weapon, the unique two-handed sword Fields of Crimson is only really useful for players investing in the Rupture skill. This unique adds ranks to Rupture and reduces cooldown for the skill on top of the unique affix that spawns pools of blood which inflict Bleeding when Rupture hits an enemy. This combination is serviceable for buffing the skill, but there are much better legendary aspects for Barbarians to use.

Ancient's Oath – Ancient's Oath can be a decent option for use in a Whirlwind build for Barbarians, but its unique affix boosts the power of the Barbarian's Steel Grasp skill which is not the ideal skill to run when compared to ones like Death Blow, except in some specific cases. This unique affix launches two additional chains of Steel Grasp and Slows enemies hit for three seconds. It does increase damage to Slowed enemies and add ranks to Steel Grasp, but these aren't enough to make this weapon useful outside a few niche situations.

C-Tier

Diablo 4 Barbarian

Hellhammer – The Hellhammer has a similar issue to Ancient's Oath in that it affects an underpowered skill, but is further held back by an even smaller pool of potential builds to use it on. While its unique affix deals additional damage via Burning enemies when the Barbarian's Upheaval skill is used, there are better Core Barbarian skills like Whirlwind, Rend, and Hammer of the Ancients that could be used instead. It can be too risky to use such a slow attack without the proper defensive affixes to back it up, making the risk of using this weapon not worth the reward.

Battle Trance – Battle Trance is a unique Diablo 4 amulet that adds additional ranks to and a unique affix for the Frenzy Basic skill. The unique affix isn't anything special as it only increases maximum Frenzy by two and attack speed by a small margin when at max Frenzy. There is really no reason to use Frenzy as a Basic skill over something like Lunging Strike, so this amulet is a pretty poor investment for most players.
